This service allows your healthcare provider to continue to track your healthcare data remotely. Our remote patient monitoring program can collect a wide range of health data from you such as:


  • Weight

  • blood pressure

  • blood sugar

  • blood oxygen levels

  • and heart rate


Your healthcare provider will monitor your vitals remotely and act on the information as part of your treatment plan. This monitoring program can help you stay connected with your healthcare provider in between visits.

How are my results sent to my healthcare provider?



By using cellular medical devices, your vital sign readings will be automatically sent to the clinical staff and monitoring system. All you have to do is register your device with our enrollment specialist via telephone and you can begin monitoring your results.


Where will I get the devices for the program?



You will be provided with state-of-the-art devices to use for the duration of the program. When the program is completed, devices are returned to your healthcare provider or to Access Managed Services.


How do I get started?



Your healthcare provider will coordinate with Access Managed Services to enroll you in the monitoring program. A clinical enrollment specialist will contact you and coordinate the receipt and registration of your new monitoring devices. The clinical specialist will review you care plan and monitoring tasks
