About Chronic Care Management
At Taylorville Family Medicine, we strive to help our patients beyond the office, especially when it comes to chronic care. We understand that when it comes to long-term medical conditions, it can become difficult to schedule an in-person appointment. That's why at Taylorville Family Medicine, we have established a chronic care management system that allows patients suffering from long-term medical conditions to receive the help they need through Access Telehealth.
Access Telehealth is a non-face-to-face service provided by Medicare beneficiaries who have two or more chronic conditions expected to last 12 months or more. With CCM, our team can continuously monitor your conditions; giving us the information we need to provide you with a tailored treatment.
