We understand that when it comes to long-term medical conditions, it can become difficult to schedule an in-person appointment. That's why at Taylorville Family Medicine, we have established a chronic care management system that allows patients suffering from long-term medical conditions to receive the help they need through Access Telehealth.
Access Telehealth is a non-face-to-face service provided by Medicare beneficiaries who have two or more chronic conditions expected to last 12 months or more. With CCM, our team can continuously monitor your conditions; giving us the information we need to provide you with a tailored treatment.

How are the results sent to my healthcare provider?
Your Vital sign readings are automatically sent to our staff through cellular medical devices
Where will I get the devices for the program?
We will coordinate shipment of these state-of-the-art devices directly to your home. When the program is completed, the devices are returned to us or to Access Telehealth.
How do I get started?
We take care of your enrollment with Access Telehealth. When you have received your devices, a clinical enrollment specialist will contact you, review your care plan and help you get started with the program.
